Hospice brings together a team of specially trained professionals and volunteers who work with the patient’s doctor to provide a plan of care woven with the dignity of choice and power of love. We have Durable Medical Equipment (DME) supplies and medications related to the patient´s hospice care. Home Health Aides provide assistance in activities of daily living including personal hygiene like grooming and dressing, bathing, and certain housekeeping.
Coordinate care with the patient’s physician and the interdisciplinary hospice team.
Coordinate care with a team of physicians, nurses, home health aides, social workers, spiritual counselors, and volunteers. Visits their patients routinely for medical support which includes medication coordination and instruction.
Social Workers counsel patients, families and caregivers to assess and coordinate needs with community resources and assist with financial matters. Social Workers can also assist with funeral arrangements.
Provide assistance in activities of daily living including personal hygiene like grooming and dressing, bathing, and certain housekeeping chores.
Provide a variety of services including counseling, bereavement and spiritual support to patients and families.
Can provide fellowship, company, and support for patients, and respite for caregivers.
Provide supplies and medications related to the patient’s hospice care.
